- Identity data such as your name, surname, Turkish national ID number, or, if you are not a Turkish citizen, your passport number or temporary ID number, place and date of birth, marital status, gender information, and a copy of the Turkish ID Card or driving licence you present
- Contact data such as your address, phone number and email address
- Financial data such as your bank account number and IBAN
- Health-related data obtained in the course of medical diagnosis, treatment and care services, such as laboratory and imaging results, test results, examination data and prescription information provided for tracking in your file, as well as your visual and audio data
- Feedback and comments you share for the purpose of evaluating our services
- Closed-circuit camera footage and audio recorded during your visit to our clinic
- Data relating to your private health insurance and Social Security Institution records, for the purposes of financing and planning healthcare services
- Medical documents, surveys, form information and location data you provide of your own free will

- Protection of public health, preventive medicine, medical diagnosis, treatment and care services
- Sharing requested information with the Ministry of Health and other public institutions and organisations pursuant to the relevant legislation

- Financing your healthcare services, covering examination, diagnosis and treatment costs, and sharing requested information with private insurance companies
- Informing you about your appointment and verifying your identity
- Invoicing in return for our services
- Verifying your relationship with institutions with which our clinic has agreements
- Responding to any questions or complaints regarding our healthcare services
- Taking all necessary technical and administrative measures for the data security of our systems and applications

Transfer of Personal Data
Within the framework of Law No. 3359 on Basic Health Services, Decree-Law No. 663, Law No. 6698 on the Protection of Personal Data, the Private Hospitals Regulation, the Regulation on the Processing of Personal Health Data and the Protection of Privacy, Ministry of Health regulations and other applicable legislation, and for the purposes explained above, your personal data may be shared with the following parties:

Method and Legal Basis of Collecting Personal Data

Under the Law and the relevant legislation, you have the following rights:
- To learn whether your personal data is being processed
- To request information if your personal data has been processed
- To access your personal health data and request such data
- To learn the purpose of processing your personal data and whether it is used in accordance with that purpose
- To know the third parties to whom your personal data is transferred, whether in Turkey or abroad
- To request correction of your personal data if it has been processed incompletely or incorrectly
- To request the deletion or destruction of your personal data
- To request that any correction, deletion or destruction be notified to the third parties to whom your personal data has been transferred
- To object to a result that is to your detriment arising solely from analysis of the processed data through automated systems
